This service may be covered partly or entirely for patients with a diagnosis of diabetes and Medicare Part B. Below are the eligibility requirements for diabetic footwear:

• o Medicare Part B coverage
• o Diabetes Diagnosis
• o Physician documentation of any of the following:

• Previous amputation of the foot or part of the foot
• History of foot ulcers
• History of pre-ulcerative calluses
• Diabetic neuropathy with evidence of callus formation
• Foot deformity
• Poor blood circulation
